Important notes on completing the application |
| |
|
Mandatory Field Entry - Most of the fields on the application require a response. You will be prompted to complete any required fields, and the system will not allow you to move forward through the remainder of the application unless completed. |

Effective Dates - Desired policy effective dates must be no earlier than the day following the application date and no later than 30 days following the application date. Any policies that are issued become effective on the date the completed application is formally approved and premium is received. |

Completion of the "Your Name" Field - The name you insert in the "Your Name" field will be the name shown on the policy as the Named Insured. This field should be completed with your name if you are an individual, followed by any doing business as (DBA) names as space permits. If you are applying for coverage for a legal entity, such as partnership, limited partnership, corporation, or limited liability corporation (LLC), the exact name of the legal entity should be entered, followed by any applicable DBA names as spacing permits. Please refer to the definitions section of the applicable specimen policy form for clarification of who is an Insured. |

Selection of Payment Method - Following receipt of a premium indication, you will be prompted to select a payment method. Following selection of a payment method, PSOnline does not allow you to return to previous screens. Therefore, call the customer service number if you have additional questions. |

Application does not Bind Insurance Company - Completion of an application does not bind the insurance company to issue coverage. While most applicants are accepted, it is possible that an applicant may not be accepted based upon the information contained in the application. |

Travel Agent Protect is designed to offer Errors and Omissions coverage to select segments of the Travel Professionals Industry. If you: |

|
are a host travel agency, a Tour Operator, or a Meeting Planner; or |
| |
|
have Total Annual Gross Receipts greater than or equal to $5M in the next twelve months; or |
| |
|
have 10% or more of your Total Annual Gross Receipts derived from student travel or adventure travel combined; or |
| |
|
maintain your own on-line booking engine on your own web site |
| |
|
have less than one year travel industry experience |
| |
then you will not qualify for coverage under the Travel Agent Protect Program. However, you may refer to www.travelproins.com where coverage may be available through a separate program. |
